If your child has participated in our ESAIP program, and you would like to provide our program with feedback, follow the steps below:

1.  To rate our program overall download the "Service Satisfaction Questionnaire," : files/Service Satisfaction Questionaire.doc 

2.  To rate the Clinical Coordinator download the  "Parent.Caregiver Clinical Coordinator Rating Scale" : files/Parent.Caregiver Clinical Coordinator Rating Scale.doc

3.  To rate individual therapists download the "Parent.Caregiver Therapist Rating Scale" : files/Parent.Caregiver Therapist Rating Scale.doc

2.  Save a copy or copies to your desktop

3. Type in your responses and comments

4.  Save the revised document(s)

5.  Go back to the website, and locate under contacts the Executive Director, Sally Grevemberg

6.  Click on the email link, then attach your revised document(s) and send
